General Summary
The Home Health Access Coordinator screens, prioritizes and triages incoming phone calls and arranges prescribed services, whether initial or ongoing, with the physician, payor, patient/ caregiver, and provider to ensure that services are provided in a timely and coordinated fashion. The position performs the duties with guidance from the Access RN and under direction of the Scheduling Supervisor.
Required
- Graduation from an accredited LPN/LVN school of nursing.
- Current license to practice in the State of Oregon as a Licensed Practical Nurse.
- Minimum 2 years experience as an LPN/LVN (Acute, Clinic, or Long Term Care).
- At least 1-2 years of Home Health experience.
- Candidate with strong computer experience and able to multi-task; triage experience and background in scheduling a plus.
At least 1-2 years of referral intake experience.
