Required Experience
  • This position develops, implements, monitors, and modifies a plan of care through an interdisciplinary and collaborative team process in conjunction with the MD, the client, and his or her caregiver.
  • Educates client and family about community support services; facilitates the goal of self-care by the client and his or her family.
  • Monitors the plan to ensure the quality, quantity, timeliness, and outcome measures of services; provides periodic reassessment to assure that services are appropriate and cost effective and do not increase the client's dependence.
Required
  • Associates degree required, BSN preferred.
  • Licensed RN in state of Kentucky.
  • Minimum of 1 year clinical nursing experience required.
  • Additional experience and/or certification for specialty areas.
Skills
  • Demonstrated ability to function autonomously within area of professional expertise.
  • Professional certification in specialty preferred.
  • Excellent oral and written skills.
  • Ability to relate to general and professional public, all levels of agency staff, and patients.
  • Thorough knowledge of VNA's patient care standards, payor requirements (i.e., Medicare Home Health Agency Manual HCFA Publication 11), and internal agency communications as they relate to issues of compliance.
